Consent form for the use of testimonials, photography and video recordings for advertising and promotional purposes

THIS CONSENT FORM IS NOT COMPULSORY.

Please only complete this form if you consent to Bridgend College, subsidiary companies of Bridgend College or organisations affiliated with Bridgend College using your testimonials, photographs and/or videos of you for the purposes listed.

Testimonials, photographs and/or video recordings can be used for any type of marketing, advertising and promotional activity (print and digital) and may include, but is not limited to, brochures, websites, posters, advertising, media outlets (i.e. newspapers, magazines) and social media.

I understand that testimonials, photographs and/or video recordings of me will only be kept for as long as necessary and will then be deleted from all college systems or archives. However, I understand that some testimonials, photographs and/or video recordings may be kept permanently once they are published and be kept as an archive of “College life”.

I understand that content on websites can be viewed throughout the world and not just in the UK, and that other countries may not provide the same level of protection to the rights of individuals as EU/UK legislation provides.

I understand that I have the right to request a copy of the Personal Data (meaning any information which is related to an identified or identifiable natural person) held about me and to request corrections or deletions of the Personal Data that is no longer required. I understand that I can ask the College to stop using my testimonials, photographs and/or video recordings of me at any time, in which case they will not be used from the point of notification to the College nor in any future publications, but they may continue to be used on any existing printed or digital materials that are already in circulation.

I understand and agree that:

copyright in the testimonials, photographs and/or video recordings will be retained by the College.
the College will have the right to edit or modify the testimonials, photographs and/or video recordings without my approval.
I will not be paid a fee in respect of any use made of the testimonial(s), photograph(s) and/or video(s).

The College is committed to processing information in accordance with the General Data Protection Regulation (GDPR) and all other applicable data protection laws. The personal data collected on this form will be held securely and will only be used for administrative purposes. If you would like to withdraw your consent to the College using any testimonials, or photographs or videos in which you may feature, or have any questions relating to this consent form or data protection, please contact the College’s Data Protection Officer: data@bridgend.ac.uk

You also have the right to lodge a complaint against the College regarding data protection issues with the Information Commissioner’s Office (ICO): ico.org.uk/concerns/

I consent to Bridgend College (the “College”) using my testimonials, or photographs and/or video recordings of me taken by the College or by an organisation authorised on behalf of the College, for the purposes of internal and external promotion or publication of the College as set out above.
